Not only did the patient get this assistance from Latino Community Association the next day, but it only took two minutes to look and see the referral was closed as opposed to having to play phone tag. This project expanded from a food insecurity focus to a broader social determinant of health focus.
Referrals were made to meet the need for clothing, food, housing, bill assistance, and family support services.
